Brazosport Regional Home Health offers a variety of services to the southern parts of Brazoria County. We provide comprehensive, quality care for patients in their own homes. Our goal is to promote our patient’s individual recovery in a more comfortable, less expensive environment of their home.
Brazosport Regional Home Health provides exceptional care, quality and services to all our clients. As part of that commitment, we operate under practice standards which are approved by the Joint Commission for Accreditation of Healthcare Organizations (JCAHO). Brazosport Regional Home Health is the only JCAHO accredited home health agency in the southern Brazoria County area. In addition to being accredited by JCAHO, we consistently receive recognition for better patient outcomes than any other local agencies on government tracked quality care issues. In fact, our home health team received the improvement award in patient’s management of oral medications from TMF Health Quality Institute, the Medicare Quality Improvement Organization for Texas. Fundamentally, our patients are reaching their healthcare goals in a significantly shorter amount of time and making fewer unplanned trips back to the hospital or emergency room.
Working with each patient and their physician, Brazosport Regional Home Health assesses the individual’s needs and abilities, develops a detailed treatment plan, and then coordinates all the necessary services to complete each plan. We provide home health services including:
